Oak-D Pro 4 Camera Problem

I received my pre-sale Oak-D Pro 4 camera and the Y-Adapter is a problem. I am trying to use the camera on Windows 11 using the SDK example app using the "camera_all" example. Everything builds fine but the camera is not found.

I believe the problem is the Y-Adapter because none of the USB-C plugs on the adapter can connect properly. The adapter cannot go into the USB-C slot in the back of the camera and the power and PC camera cable cannot be inserted all the way into the back of the Y-Adapter. I have worked with USB-C for years and I have never seen a USB-C connector that does not go in all the way but with the Y-Adaper, none of the USB-C connectors can go in.

Can anyone explain what I should do? I have read that PoE might work but all of my RGBD cameras (all the major vendors - RealSense, Azure, Orbbec) just connect to USB-C. The Y-Adapter came with the camera so I don't have any alternatives to try there.

Sorry to hear you’re having trouble with your camera. It sounds like the Y-adapter isn’t seating fully in your Oak-D Pro 4. Here’s a picture of how it should look:

There’s a small red light on the Y-adapter that turns on when power is connected — is that light on?
Could you also try connecting the camera directly to your PC using a USB3 (blue) USB-C cable, bypassing the Y-adapter, and see if it’s detected? Most PC hosts can power the camera directly over USB.

I am just now seeing this. The camera in the image is a Oak D Pro - not an Oak 4 D Pro. I tried using the Y-connector on the Oak 4 D Pro and it didn't help. I bought a NetGear GS305P POE+ switch to get the power to the Oak 4 D Pro and I am getting a blinking blue light on the front. This is supposed to indicate that it is ready for setup. When I open it in the Oak Viewer it finds the device but hitting "Connect" reports that the device is not setup. I tried "Oakctl start-setup" without success as well. Error message was "Incoming stream encountered an IO error."

Yikes this is hard but I am determined to get it working. Any advice on how to proceed?

Hey @bobdavies2000 - I misread what you wrote, my bad...
So for OAK 4 Pro I would suggest you follow our guide for getting started:

The recommended way to connect to your camera is through PoE+ cable, the Y-connector is not supported.
If you want to connect with ADB you need to power camera with PoE+ also...
If your device is old, I would suggest updating the software with oakctl, you can find how to on the page I shared under troubleshooting/Blue light does not behave as expected.

Oskar - thanks for the link. I reviewed it and the difference from my experience is that when I connect the camera to the the POE+ switch, I get a blinking blue light instead of a solid blue. However, the Oak Viewer does see the device. When I click on the device, I get 2 messages - Device <IP address> is not setup. The messages have a "Report Error" which I clicked on to send an email reporting the error.

Thanks for the info on the USB connection. I won't bother trying to get that path working but I am stuck on getting the POE access to work. The camera was an early release version and may not behave the same as the latest versions. I tried the oakctl approach as well and got the error:

"incoming stream encountered an IO error (TLS connect): tls handshake eof".

So the 2 methods of getting the camera to work are both failing. Any recommendations?

I should have added that the Oak Viewer finds and uses my Oak 3 D Pro camera working. But the problem is with this Oak 4 D Pro camera:

It should be noted that the link still shows that the USB3 is supported. I had purchased the F-Sync Y connecter thinking that was all I needed.

I just found the reset button and tried that only to find that the camera now has no light showing on the front. Is there a process describing how to use the reset button?

So a blinking blue light instead of a solid blue one means you have an older version of software installed on the camera...
You will have to do a system update, follow the tutorial here:

First try with oakctl. You can run oakctl device update, if it doesn't work, you will need to perform the Full OS Flash, which is also documented in the link I send...

I read that I should hold the button for 10 seconds and I tried that as well so I should be back to the factory reset. The light did not return so I disconnected from the 30W POE switch after which the flashing blue light came back. Any ideas?

I have a blue flashing light and with the USB device connected to the computer I see "Oak4-D R7" in the device manager. When I try to use "oakctl setup", it says the "incoming stream encountered an IO error (TLS connect)". If I try "oakctl setup -d 192.168.1.188", it says that the incoming stream encounterd an error (TCP connect): No connection could be made because the target maching actively refused it"

My serial number has "2518" in it - less than the "2536" base model. The camera was an early release camera made in week 18 of 2025.

I used Gemini and Copilot suggestions to hit the reset button and flash the flash the full OS but I cannot get that to work. Without the POE connected, no light shows up on the front of the camera.

I download the depthai-sdk and looked for the depthai-bootloader. I found several things that looked like they were flashing the full OS but trying them did not work either.

I would suggest trying to find an oakctl that worked with the early release cameras and using it to get flash the full OS and start again. It might be easier to just ship me another camera and I send this one back and you can try all the tricks available.

The camera is a fixed focus version but I don't think that makes a difference.

I don't know how to put the device into EDL mode. That sounds promising

As soon as I connected the usb-c to usb-c cable, the blinking blue went solid! Progress! The "oakctl version" gives 0.16.5 but the oakctl start-setup gives "There are no available connections". Trying the Oak Viewer, gets the same message.

I tried adding the POE and the results were similar - Incoming stream encountered an IO error (TCP connect): A connection attempt failed because the connected party did not properly respond after a period of time, or established connection failed because connected host has failed to respond. (os error 10060)

The camera is now back to a blinking blue so I tried disconnecting and reconnecting the usb-c cable. The camera is occasionally flashing red.

You need to perform an update...
So plug in just usb-c to usb-c cable to your OAK, then one or other:
Option 1: Flash using URL

oakctl device flash https://luxonisos-rvc4.fra1.digitaloceanspaces.com/PRODUCTION/luxonis_os_rvc4-1.23.0.tar.xz

Option 2: Flash using a downloaded file
First, download the full-flash .tar.xz image (latest 1.23.0, other versions available in Changelog) and extract it (e.g. with 7-Zip/WinRAR/tar).
Perform the full OS flash using oakctl and the path to the downloaded file.

oakctl device flash <path-to-downloaded-image>

Oskar - thanks for the response. The results are below. I am pretty sure that these unexpected results are from being an early release unit. I will jump on any further suggestions.
